Apply For Services
Individuals interested in receiving services at Successful Living must have an eligible funding source, a diagnosed mental illness, and an assigned case manager. If you meet these requirements, please complete our application for services below.
What Comes Next?
1. Application Review
Once an application is received, the Admissions Manager will review the information provided. If additional details or documentation are needed, the applicant, client, guardian, or case manager may be contacted.
2. Intake Interview
If the applicant appears to be a good fit for services, an intake interview will be scheduled with the client, case manager, guardian (if applicable), and members of the Successful Living team.
During the interview, the team will discuss:
The program being requested
Other services that may be beneficial
Mental and physical health history
Daily living needs and supports
Hospitalization, legal, and support history (if applicable)
Current needs, preferences, and recovery goals
3. Admission Decision
Following the interview, the Admissions Manager will review all information and make a final determination, typically within two business days.
4. Service Planning
If approved for services, an Individual Service Plan (ISP) meeting will be scheduled with the client and support team. During this meeting, personalized goals, support needs, and safety planning will be reviewed to create a plan for service delivery.
