Whether you're planning a hospital discharge, assisting someone transitioning back into the community, or helping an individual maintain independence, our team is here to make the referral process simple and responsive.
Who We Serve
Successful Living provides services for adults (18+) living with chronic mental illness who could benefit from additional community-based support.
Our services may be appropriate for individuals who:
Are living with a chronic mental illness.
Need support maintaining independent living.
Are transitioning from a hospital, residential treatment, incarceration, or another level of care.
Would benefit from structured daytime programming.
Need assistance developing life skills and community connections.
Are interested in psychiatric rehabilitation, peer support, or supported community living.

How to Refer
Step 1.
Complete our referral form or contact our Admissions Manager.
Step 2.
Our staff will review the referral and determine eligibility.
Step 3.
We'll contact the individual (or referral source when appropriate) to begin the intake process.
Step 4.
Once eligibility is confirmed, services can begin.
